How to nominate

You can nominate yourself or someone can nominate on your behalf (with your consent). This extends to PBB staff putting you forward as a nominee.

Simply send the following nominee's details to heart@petbloodbankuk.org or by post to Heart of PBB Award, Pet Blood Bank UK, Units 21 & 22, Loughborough Technology Centre, Epinal Way, Loughborough, Leicestershire, LE11 3GE

• Full name
• Address
• Contact telephone number
• Email
• Photo (if available)
• A few words outlining your main PBB achievement and why you got involved
• Other PBB achievements (if applicable)
• Closing with why you/ they should be the Heart of PBB

Please find attached a nomination form that you can download and fill in, and either email, or print off and post to us.

We would like to use both names and images of winners and nominees for publicity purposes, helping us to spread the good word about the work we do. Please let us know on your nomination if you are not happy to take part in this.

All nominations to be received by 31^st December 2013.
